Care plans are at the heart of delivering quality care. Skedulr makes it easy to create detailed, shareable plans that carers can access on every visit.
Step 1: Navigate to Clients
Go to Clients in the sidebar and select the client you'd like to create a plan for, or add a new client first.
Step 2: Open Care Plan
Click the Care Plan tab on the client's profile page. Then click Create Care Plan.
Step 3: Add Sections
A care plan typically includes:
- Personal Details — name, date of birth, emergency contacts
- Health Conditions — current diagnoses, medications, allergies
- Daily Tasks — morning routine, meals, personal care, mobility
- Preferences — how the client likes things done
- Risk Assessments — falls risk, skin integrity, medication management
Step 4: Assign & Share
Once your plan is complete:
- Click Save & Publish
- The care plan becomes available to all carers assigned to this client
- Carers can view it in the mobile app before and during visits
Keeping Plans Up to Date
- Review care plans at least monthly or after any significant change
- Use the revision history to track what changed and when
- Carers can flag updates from the mobile app if they notice something during a visit
